Sentara Medical Group is now hiring a Full-Time Social Worker (MSW) in Suffolk, VA!

This position covers Hampton Roads (Western Tidewater) and Northern Virginia and requires travel to homes and clinical visits. Travel to Northern Virginia is not required.

THIS POSITION IS HYBRID! 2-3 DAYS/WEEK WORKING IN OFFICE, REMAINING DAYS WILL BE REMOTE.

Hours: Monday-Friday, 8 am - 4:30 pm

As a MSW with SMG, you will be responsible for the provision of medical social work services to patients receiving care in the hospital setting. Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs, for each assigned patient. Collaborates with patients, families, healthcare team members, and community agencies to develop and implement plans to address identified needs. Evaluates effectiveness of plans and initiates change as needed.

Qualifications:

  • Master's of Social Work required
  • 2 years Social Worker experience required
  • 2 years Acute Care experience required
  • Community Based Care highly preferred
  • Ambulatory experience highly preferred

Job Summary

Responsible for the provision of medical social work services to patients receiving care in the hospital setting. If working in one of the mental health acute care units, 1yr experience in psychiatric setting required.

MLD - Master's Level Degree: Social Work (Required)
